In situations where ESA requests a doctors’ note or when an employee is required to provide a completed Major Medical Absence Report (MMAR), ESA will reimburse the employee up to a maximum of six (6) notes per calendar year to a maximum annual value of $200.00 towards the actual cost of such note(s) or MMAR(s).
In the event MMAR or ▇▇▇▇ are required to disclose any Confidential Information pursuant to judicial order or other legal mandate, MMAR and/or ▇▇▇▇ shall promptly notify the Company to enable the Company to seek a protective order or similar remedy to prevent such disclosure.
If the Company is unable to obtain a protective order or similar remedy, MMAR and ▇▇▇▇ agree to only disclose that portion of the Confidential Information that is required to be disclosed and to maintain the confidentiality of any other Confidential Information.
To receive reimbursement, MMAR must submit appropriate supporting documentation (e.g., receipts, proof of payment, etc.).
Bloomberg has informed EY that it wishes to make available a copy of the Report in connection with the Services to the Recipient.
