Insurance Application means the document provided by the Insured to the Insurer prior to the Policy Period, as set forth in Exhibit C.

Insurance Application. A teacher on leave under Sections 11, 12, and 13 of this Article is eligible to continue to participate in group insurance programs if permitted under the insurance policy provisions, but shall pay the entire premium for such programs that he/she wishes to retain commencing with the beginning of the leave.
