These include, but are not limited to: - Imaging, laboratory services, and professional services - In vitro fertilization (IVF) - Zygote intrafallopian transfer (ZIFT) - Gamete intrafallopian transfer (GIFT) - Cryopreserved embryo transfers - Gestational carrier cycles - Any related services, products or procedures (such as intracytoplasmic sperm injection (ICSI) or ovum microsurgery).
Gamete storage is not funded through transgender health services.
This includes, but is not limited to, the following: a) procedures: invitro fertilization; embryo transfer; embryo freezing; and Gamete Intra-fallopian Transfer (GIFT) and Zygote Intra-fallopian Transfer (ZIFT); donor sperm, surrogate motherhood; b) Prescription Drugs not eligible under the Prescription Drugs section of the Contract; and c) ovulation predictor kits.
However, the following services and supplies are not covered: • Gamete or embryo storage • Use of frozen gametes or embryos to achieve future conception • Pre-implantation genetic diagnosis • Donor eggs, sperm or embryos • Gestational carriers (surrogates) Genetic testing is covered when determined by Health Net to be Medically Necessary.
This benefit covers one (1) egg harvesting and up to two (2) transfers through InVitro Fertilization (IVF), Zygote Intra-Fallopian Transfer (ZIFT), or Gamete Intra-Fallopian Transfer (GIFT) only, during each twenty-four (24) month period from the date of the first visit for actual treatment from the participating ART Specialist and after the determination of eligibility as described above.
