Final Evaluation Report definition
Final Evaluation Report means the final formal evaluation document to be signed by both the unit member and the administrator and placed in the unit member’s personnel file (Form F, Form F-NC, or Form H).

(4) For those members of the Monroe City Education Association MEA/NEA whose employment is NOT regulated by 1937 (Ex Sess) PA 4, MCL 38.71 to 38.191: Any teacher found to have misrepresented, violated or abused his sick leave and/or temporary leave provisions of this Agreement, may be denied compensation for those days and may be disciplined in accordance with just cause.
(5) For those members of the Monroe City Education Association MEA/NEA who are NOT teachers, as defined in section 1 of article I of 1937 (Ex Sess) PA 4, MCL 38.71:
Final Evaluation Report. Not less than 30 calendar days before the last regularly scheduled school day of the employee’s scheduled work year in which the evaluation takes place, the evaluator shall prepare and issue the Final Evaluation Report in which the employee’s overall performance and progress toward objectives is evaluated. The ratings for a Final Evaluation shall be “Exceeds Standards”, “Meets Standards”, and “Below Standard”. Prior to the end of the school year the evaluator shall hold a Final Evaluation Conference with the employee to discuss the content of the Final Evaluation Report. When a Final Evaluation Report is marked "Below Standard," the evaluator shall specifically describe in writing the area(s) of below standard performance, together with recommendations for improvement, and the assistance given and to be given.
6.1 The employee's required signature on the form does not necessarily indicate agreement with its contents, and the employee may attach a written response to each copy of the form within ten working days from date received. Such a written response becomes a permanent part of the record. The employee may also appeal the matter to the next higher administrative level.
6.2 The employee shall be given a copy of the Final Evaluation Report at the conference. A copy shall be placed in the employee's personnel file, and a copy retained by the school or office. Evaluation forms shall not be considered a public record.

Grantee must prepare a Project Director’s Final Evaluation Report (“Final Report”) in accordance with the Evaluation Plan described in Exhibit A and in the form provided by Agency to Grantee.
The evaluator and evaluatee shall hold a final summary conference to discuss the Final Evaluation Report.

More Definitions of Final Evaluation Report
Final Evaluation Report shall be based on evaluation criteria as defined in Article 11. The unit member will receive a rating for each of the six (6) California Standards for the Teaching Profession (CSTP) that may or may not include evidence for each of the elements. Final Evaluation Report means the document reporting the results of the evaluation, which becomes a part of the employee’s personnel file (Appendix C).
Final Evaluation Report shall have the meaning given to it in Section 2.3.1(e).
Final Evaluation Report means the report described in Section 6.08(c).
Final Evaluation Report means a report, in the form set out in Annex D to the foregoing offer, assessing the delivery of the Grant Outcomes which includes a summary of the grant-aided activity undertaken, an assessment of progress towards or success in achieving the Grant Outcomes and a summary of any lessons learned and recommendations for the future.

The final report shall be reduced to writing on the Teacher Evaluation Form agreed to between the association and the district. • Following the final observation, both the teacher and the evaluator will check the appropriate rubric for each component included in the evaluation and a conference held within 10 days to discuss the similarities and differences. The teacher will have a final opportunity to provide documentation to support any area where there are differences. • A copy of the final written evaluation shall be given to the teacher within 10 working days following the completion of the above conference. • The final written evaluation should contain only information previously known to or discussed with the teacher. • The teacher is required to sign the evaluation form and will receive a copy for their records. • If a teacher disagrees with the result of an evaluation report, he/she may submit a written explanation for attachment to the personnel file copy. • If a teacher is denied continued employment, the Board will advise the teacher of the reasons, in writing, with a copy to the association, and provide a hearing where requested. • Probationary teachers shall receive a written recommendation within 60 days prior to the end of the probationary year(s) specifying renewal or non-renewal of probationary contract or tenure recommendations. • Non-renewal of probationary teachers shall be governed by existing tenure statutes. Failure to follow the steps of this process shall be subject to grievance.
Final Evaluation Report means that document which becomes a part of the employee's personnel file. The Employee Final Evaluation Report (Short Form) is attached to and made a part of this Agreement as Appendix E. The Educational Support Staff Final Evaluation Report (Short Form) is attached to and made a part of this Agreement as Appendix F. The Employee Final Evaluation Report (Long Form) is attached to and made a part of this Agreement as Appendix H. The Educational Support Staff Evaluation Report (Long Form) is attached to and made a part of this Agreement as Appendix I.
