This Business Associate Agreement (“BAA”) is entered into on the Effective Date, by and between Medical Hearing Consultants, LLC (“Business Associate”), and Health Care Provider (“Covered Entity”).

As the common law employer of the Consultant(s), Experis is responsible for making offers of employment, hiring, conducting performance reviews, employment termination, resolving issues that may arise from the employment relationship with the Consultant(s) and enforcing Experis’ employment policies relating to Consultant conduct at the worksite.
