Authorized Claimant definition

Authorized Claimant means any Class Member whose claim for recovery has been allowed pursuant to the terms of the Stipulation.

Authorized Claimant means a Class Member who submits a timely and valid Proof of Claim form to the Claims Administrator.

Authorized Claimant means a Settlement Class Member who submits a Proof of Claim Form to the Claims Administrator that is approved by the Court for payment from the Net Settlement Fund.

• If you did not submit atimely proof of claim and release pursuant to the 2016 Notice, 2017 Notice, 2018 Notice, 2019 Notice, or 2022 Notice, then to participate in and receive your share of the Net Settlement Fund, you must submit a valid and timely Proof of Claim and Release demonstrating that you are an Authorized Claimant as set forth in the Settlement Agreement.

• Within ninety (90) calendar days after the mailing of the Notice or such other time as may be set by the Court, each Person claiming to be an Authorized Claimant shall be required to submit to the Claims Administrator a completed Proof of Claim, substantially in the form of Exhibit A-3 attached hereto, signed under penalty of perjury and supported by such documents as are specified in the Proof of Claim.

• Distribution payments must be made by check or electronic payment payable to the Authorized Claimant (beneficial account owner).

• If the prorated payment to any Authorized Claimant calculates to less than $10.00, it will not be included in the calculation and a distribution will not be made to that Authorized Claimant.
