WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT. I understand that the education and work of a Certified Nursing Assistant, including lab work and clinical rotations within nursing facilities or other healthcare facilities with which I may be associated, are inherently dangerous and could expose me to accident and injury and even death, including but not limited to, blood-borne and air-borne pathogens, needle sticks, and many other dangerous and hazardous situations and environments. As such, in attending any NWNACT training event, lesson, or session, I hereby release and hold harmless Northwest N.A.C Training, and any of their employees, instructors, and volunteers (herein called Staff) from any and all liability, claims, demands and actions whatsoever, associated with any risks incurred therein arising from or relating to any injury or damage, including death, that may be sustained by myself or property belonging to me even if caused by negligence on the part of NWNAC Training staff, while participating in any NWNACT activity on or around the premises where the NAC Program is being conducted

WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT. I certify that I am the authorized representative of the organization/individuals named herein. I agree on behalf of myself and the organization to supervise all activity on the premises and to fully comply with and enforce the Palacios Recreation Association rules and regulations during the time allocated for use by the organization/individual and guests. This agreement may be immediately terminated if the organization/individual and/or guests fails to comply with and/or enforce ▇▇▇▇▇▇▇▇ Recreation Association rules and regulations. In consideration for the acceptance of this application, the organization/individual and guests named herein hereby waives, releases and agrees to hold harmless the ▇▇▇▇▇▇▇▇ Recreation Association, its officers, officials, employees, agents and volunteers including class instructors from any and all claims for injuries, damages, losses or suits, including all legal costs and attorney’s fees, arising out of or in connection with the participation in this agreement and the rental of this Palacios Recreation Association facility by the organization/individual and guests named herein. WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT. This is a legally-binding Liability Release, Waiver, Discharge, and Covenant Not to Sue made by me, (hereinafter referred to as “Participant”) to THE BOARD OF TRUSTEES OF EASTERN ILLINOIS UNIVERSITY (hereinafter referred to as the “University”).

WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT. In consideration for participating in the GHSA “3-Point Shooting” contest, I hereby release, waive, and discharge not to sue the GEORGIA HIGH SCHOOL ASSOCIATION AND OFFICIERS, HOST VENUES/FACILITIES, CONTEST OFFICIALS, AND EMPOYEES from any and all liability, claims, demands, actions, and causes of actions whatsoever arising out of or related to any loss, damage, injury, or death that may be sustained to me, or any property belonging to me, WHETHER CUASED BY THE NEGLIGENCE OF THE RELEASES, or otherwise, while participating in this activity, or while on the premises where the activity is being conducted or in transportation to and from the venue/facility.

WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT. In return for Duke University allowing me to participate in this activity and having read and understood this Participation Agreement, I hereby state that I voluntarily agree to the following:

WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT. I am a Uniformed Transit Operator for the City of Yakima who must maintain a valid Commercial Driver's License (COL) in order to operate any commercial motor vehicle. I understand that I must be physically qualified to do so in accordance with the Physical Qualifications for Drivers standards as set forth in 49 CFR 391.31, and must obtain a medical examiner's certificate initially and for subsequent license renewals. The City of Yakima provides paid medical examination services for this purpose. As an alternative to the City provided examination, I am electing to utilize my regular physician for the screening and certificate at my own expense.

WAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT. In consideration for the services provided by Southern Vines Texas, LLC, I hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE, Southern Vines Texas, LLC, their officers, agents, independent contractors, or employees (hereinafter, “Releasees”) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of, or related to, any loss, damage, or injury (including death) that may be sustained by me, my family, and/or group members utilizing the service, or any of the property belonging to me, my family, and/or other group members utilizing the service, whether caused by the negligence (other than gross negligence) of the Releasees, or otherwise, while participating in such activity, or while in, on or upon the premises where the activity is being conducted.
