Treatment Program. 22 Treatment Program shall be in accordance with CONTRACTOR’s Program 23 Statement. 24 Minimum standard: Contractor shall facilitate at least weekly individual and group 25 therapy for all Xxxxxx Youth/NMDs and regular family therapy as indicated by Social 26 Worker/Probation Officer. 27 CONTRACTOR shall follow the treatment program based upon the type of facility 28 license held and as described in their approved Program Statement.
Treatment Program. Treatment Program is considered to have the following components: • Inpatient or outpatient treatment, or a combination of both • Treatment aftercare program • EAP case management for up to a year following treatment. A treatment program is considered ended when all three of the above have been completed or when an employee terminates participation in any of the components. Treatment will be covered if it is provided by one of EAP's recommended facilities. If these facilities are not used, coverage will be limited to that normally covered under the employee's medical benefits plan.
Treatment Program. If an employee receives a positive test result, the employee will be requested to undergo drug and/or alcohol testing if the employee has been referred by Allina for chemical dependency treatment. The employee may be requested to undergo drug and/or alcohol testing without prior notice during the evaluation or treatment period and for a period of up to two (2) years following the referral for prescribed chemical dependency treatment.

At CICOA’s sole discretion and in accordance with applicable state law, it might require an Employee to undergo drug and alcohol testing if the Employee has been referred for chemical dependency treatment or evaluation or is participating in a chemical dependency treatment program. In these circumstances, the Employee may be required to undergo drug and alcohol testing without prior notice during the period of evaluation or treatment and for a period of two (2) years following completion of any prescribed chemical dependency program.
Treatment Program. Includes one pair of customized shaping lenses and a TOTAL of 4 follow-up evaluation visits for the first year. The cost ranges from $2800-$3000 and is based on customized shaping lens parameters and fit complexity. Payment is required at time of service. Where a prescription range is -5.00 or Higher and/or requires a dual axis fit, additional fees (up to $600) will be charged.
Treatment Program. The residential program provides intensive, long-term substance abuse treatment services for women ages 18 and up and their families on a 24-hour basis. The program offers the opportunity for pregnant women to return to the treatment facility with their newborn infants following delivery. The Agency shall provide the location of services and hours to the CCC for approval, prior to the commencement of program services. The Agency will document that outputs/units of service have been provided. Agency’s Performance Measurements, approved by the Citizens’ Commission for Children (CCC) will be documented and reported upon bi-annually.
