SHIFTING of teeth might occur after braces are removed. For this reason, retainers are constructed which must be diligently worn for a period of time which will vary between patients. Retainers are made of materials that are subject to breakage no matter how well constructed. Retainers must be handled and used carefully. Repair charges may be made. Instructions will be given concerning these appliances. I recognise that it is my responsibility to follow instructions completely and seek attention in a timely manner should any unexpected problems occur by informing Fairmilehead Dental Practice immediately. I must explicitly follow any instructions, either written or oral, which have been given to me relating to this orthodontic treatment. INFORMED CONSENT I have been given the relevant information, including leaflets, and the opportunity to ask any questions regarding the nature and purpose of orthodontic treatment and have received answers to my satisfaction. I have been given the alternative of seeking care with an orthodontic specialist. I do voluntarily assume any and all possible risks, including risk of substantial harm, if any, which may be associated with any phase of this treatment in the hope of obtaining the desired potential results, which may or may not be achieved. No guarantees or promises have been made to me concerning any results from treatment. The fee(s) for these services have been explained to me and I accept them as satisfactory. By signing this form, I accept all terms and conditions expressed within it and freely give my consent to authorise Dr and any and all associates necessary in rendering services that he/she deems necessary or advisable for this subject orthodontic treatment. SHIFTING. Within berth shall be charged one and one-half times the Zone rate. Shifting between two berths shall be charged as an undocking and a docking.

SHIFTING. Charterer shall have the right to shift Vessel within any port of loading and/or discharging from one loading or discharging place back to the same or to another such place once or more often. In the event that Charterer exercises this right, Charterer shall pay all additional expenses properly incurred, including additional Bunkers. Time spent shifting shall count as laytime or, if Vessel is on demurrage, as time on demurrage. For purposes of freight payment, the places grouped in port and terminal combinations in WORLDSCALE are to be considered as berths within a single port, with Charterer paying shifting expenses in accordance with the foregoing.

SHIFTING. Within terminal or from Baltimore Anchorage to/from a terminal shall be charged one and one-half times the Zone rate. Moving between two berths shall be charged as an undocking and a docking. If more than one zone in the shift is involved, the applicable rate shall be the rate for the higher zone.
SHIFTING. Tug service performed moving barges, scows, lighters, and other Vessels without propelling power, whether or not including shifting with a docking or a sailing at steamship piers, shall be charged at the hourly rate plus applicable travel charges. Whenever a Vessel having use of its own propelling power is shifted from berth to berth, the charge for the Tug(s) shall be one and one half (1½) the higher base Zone Charge, plus one and one half (1½) the applicable Tonnage Charge.
