Services Ordered. 1. AllyNet for Triage to Telemedicine (Software License) Built with EMS' high standards of care and needs in mind, MD Xxxx's telemedicine software, AllyNet, is unlike any other. It puts real-time, virtual care at the fingertips of EMS dispatch and response personnel to triage non-emergency callers to qualified healthcare professionals. AllyNet optimizes the overall triage experience for dispatchers, patients, and providers by: • Incorporating familiar, PSAP one-button transfer functionality for both dispatchers and providers to easily triage patients to providers and back to dispatchers if an emergency ever arises. • Enhancing traditional telemedicine with information already-captured in EMS systems to avoid repetition, decrease call handling times, and empower providers with better information to deliver better patient care. 2. AllySupport for EMS Triage (Provider Support) Dedicated, virtual medical triage support that is bolstered by AllyNet's unique functionality and answered on the first call to ensure the best patient experience and outcomes possible. Subscription Term (initial): One (1) year from Implementation Date. Renewal Subscription Term: Two (2) years from the end of the initial Subscription Term. Authorized End Users: MD Ally will grant access to an unlimited number of Authorized End Users (“User(s)”); provided that each User must be (i) an employee or contractor of the entity identified above as Customer, or (ii) an affiliated agent or healthcare professional with permissions from Customer to access the Services pursuant to Customer’s rights under the related Agreement. Use: MD Ally will grant unlimited access and use of Services to Users for internal business purposes only. Services Ordered. The Customer subscribes to Services pursuant to this Agreement for the sole purpose of resale. The Customer may not utilize any Service or portion thereof for its own use or that of its parent, subsidiaries, or affiliates, except on an incidental basis; provided, however, this section shall not preclude the resale of Services to another certified reseller by Customer. The Customer subscribes to those Services in the volumes stated in Attachment 1 on a monthly basis for the Service Term. Except as provided elsewhere in this Agreement, the Customer agrees to have in service a monthly average of Local Exchange Access Lines as stated in Attachment 1 for each year of the Service Term ("Volume Commitment"). As of the date of execution of this Agreement by both parties, the Company shall only provide such Services in New York State. Such Service shall be provided pursuant to New York Telephone Company's PSC 915 Tariff, which is incorporated by reference as if set forth fully herein, and pursuant to the terms and conditions set forth in this Agreement.

Services Ordered. Hyperion shall specify each specific GTE Service ordered for each Hyperion end user customer: i.e., GTE will not transfer Services “as is.” GTE will not release information to Hyperion on GTE end user customer accounts unless Hyperion first provides to GTE a written Letter of Authorization, signed by the end user customer, authorizing the release of such information to Hyperion.
