Service Coordinator. In order to monitor, coordinate and facilitate implementation of the terms and conditions of this Agreement, each Party shall nominate a representative to act as the contact person for the provision of the Services (each such representative, a “Service Coordinator”). The Service Coordinators will meet as reasonably necessary to carry out the terms and conditions of this Agreement, but no less than once per quarter during the term of this Agreement, to discuss any matters relating to this Agreement and to monitor the progress of GWB’s migration planning pursuant to Section 2.9. Each Party shall have the right at any time, and from time to time, to replace its Service Coordinator by advising the other Party in writing of such replacement. The Parties agree that the Service Coordinators will be the initial point of contact for each Party responsible for monitoring and coordinating the provision and receipt of Services under this Agreement. Each Party may treat an act of the Service Coordinator nominated by the other Party as an act of and authorized by the other Party without inquiring behind such act or ascertaining whether such Service Coordinator had authority to take such act.
Service Coordinator. The County and the City each shall designate in writing thirty days prior to the expected Commencement Date a person to transmit instructions, receive information and otherwise coordinate service matters arising pursuant to this Agreement (each a “Service Coordinator”). Either party may designate a successor or substitute Service Coordinator at any time by notice to the other party.
Service Coordinator. Each party will nominate in writing a representative to act as the primary contact with respect to the provision of the Services (each such person, a “Service Coordinator”). The initial Service Coordinators are set forth in Exhibit B attached hereto. The parties shall notify each other promptly in writing of any change to their respective Service Coordinator, setting forth the name of the replacement and stating that the replacement Service Coordinator is authorized to act for such party in accordance with this Section 1.9. The Service Coordinators shall have the authority to manage the services contemplated under this Agreement.
Service Coordinator. The person with primary responsibility for providing Service Coordination and Care Management to STAR+PLUS MMP Enrollees. The Service Coordinator leads the Service Coordination Team and actively collaborates with the Enrollee’s specialty care Providers, including behavioral health and LTSS service Providers, as appropriate.
Service Coordinator i. Contractor’s Service Coordinator will lead the triage and ticketing processes, serving as an escalation point for unresolved P1 and P2 issues.
Service Coordinator. Purchasers and Service Provider shall nominate in writing a representative to act as the primary contact with respect to the provision of the Services and the resolution of disputes under this Agreement (each such person, a “Service Coordinator”). The initial Service Coordinators shall be Xxxxxx Xxxxxxxxxxx for Purchasers and Xxxxxx Xxxxx for Service Provider (or their designated delegates). Unless Service Provider and Purchasers otherwise agree in writing, Service Provider and Purchasers agree that all notices and communications relating to this Agreement other than those day-to-day communications and xxxxxxxx relating to the actual provision of the Services shall be delivered to the Service Coordinators in accordance with Section 7.3 hereof. The Service Coordinators shall use their commercially reasonable efforts to meet as expeditiously as possible to resolve any dispute hereunder.
Service Coordinator. The Service Coordinator will coordinate the services of the case managers and other professionals providing services to residents. The Service Coordinator will also provide information and referral services to residents, including coordinating classes held in the Community Building on topics such as cooking, health care, and medical management. The Service Coordinator will also provide crisis intervention as needed and respond to the interest of various groups and individuals who desire to volunteer to visit and/or assist residents. The Service Coordinator is anticipated to live at the Apartment Complex and serve as the on-site property manager. The Service Coordinator will ensure the consistent availability of a case manager for each resident, which case manager will provide independent living services (“Independent Living Services”) to the resident. Independent Living Services will be provided and monitored by the Regional Center of Orange County (“RCOC”), which coordinates support services to individuals in independent living settings. RCOC focuses on functional skills training for adults who generally have acquired basic self-help skills or who, because of their physical disabilities, do not possess basic self-help skills, but who employ and supervise aides to assist them in meeting their personal needs.
Service Coordinator. Franchisee shall designate in writing on or immediately following the Franchise Date a person to transmit instructions, receive information and otherwise coordinate service and community relation’s matters arising pursuant to this Franchise Agreement (each a "Service Coordinator"). The City Service Coordinator shall be the City Manager. Either party may designate a successor or substitute Service Coordinator at any time by written correspondence to the other party.
Service Coordinator b) Two (2) or more Service Coordinators who can split the shift
Service Coordinator. Each of the Parent and the Purchaser shall appoint a representative to act as the primary contact person with respect to the performance of the TSA Services (each, a "Service Coordinator"). The initial Service Coordinators, including relevant contact information (including business addresses, email addresses, telephone numbers and facsimile numbers), are set forth on Schedule 8(a). Schedule 8(a) shall also include the name and relevant contact information for each Service Coordinator's backup, who shall perform the Service Coordinator's duties if the Service Coordinator is not available. Each party may replace its Service Coordinator (or the backup Service Coordinator) with an employee or officer with comparable knowledge, expertise and decision-making authority after reasonable consultation with the other party as to the proposed replacement by providing an Operations Notice, and such change shall be effective upon the other party's receipt of such Operations Notice.
