Reporting Misconduct. 163. APD shall require that all officers and employees report misconduct by any APD officer or employee, including themselves, to a supervisor or directly to the Internal Affairs Division for review and investigation. Where alleged misconduct is reported to a supervisor, the supervisor shall immediately document and report this information to the Internal Affairs Division. Failure to report or document alleged misconduct or criminal behavior shall be grounds for discipline, up to and including termination of employment.

Any individual participating in provision of IDEA/Part C services is required to report misconduct to IDEA/Part C State Office within five (5) working days by way of a formal, written complaint (see procedures for Family Rights and Safeguards). If at any point, any individual who reasonably believes that an EIS provider is posing an imminent risk of danger to children, parents, or staff should report the information to a local law enforcement agency or South Carolina Department of Social Services and then to IDEA/Part C State Office within twenty-four (24) hours.

Where apparent misconduct is reported to a supervisor, the supervisor shall immediately document and report this information to the IAO. The default discipline for a failure to report criminal behavior shall be termination.
Reporting Misconduct. Church Personnel have a duty to report their own ethical or professional misconduct and the misconduct of others. Church Personnel must hold each other accountable for maintaining the highest ethical and professional standards. When there is an indication of misconduct by Church Personnel, the person who learns of the misconduct should notify his/her supervisor or other appropriate Church authorities.

Reporting Misconduct. 17.1 The Executive shall report to the Board any acts of serious misconduct, dishonesty or material breach of Company rules committed by any employee of the Company of which he is aware or which he is aware are contemplated. Any failure to do so may be regarded as gross misconduct depending on the circumstances.
