Portal Access. Patient shall sign up for an account in the secure portal provided by Clinic, through which all non-urgent communications can be addressed. Such communications shall be dealt with by the appropriate physician or staff member of Clinic in a timely manner, but will not be monitored after-hours or on weekends. Matters requiring more immediate attention should be communicated by telephone. Patient understands and agrees that email and the internet should never be used to access medical care in the event of an emergency, or any situation that Patient could reasonably expect may develop into an emergency. Patient agrees that in such situations, when a Patient cannot speak to Clinic personnel immediately in person or by telephone, that Patient shall call 911 or go to the nearest emergency medical assistance provider, and follow the directions of emergency medical personnel.
Portal Access. We will host and provide re:SearchTX to allow Users to search Metadata stored within re:SearchTX that directs Users to Documents you store outside of re:SearchTX. At no cost to the Client, you will provide us with access to the Documents to the extent necessary for us to perform our obligations under this Agreement.

If the Customer elects to use the portal, Blackboard grants for the Term of such use to Customer a limited non-exclusive, worldwide, royalty-free license to place a digital image of the Blackboard "sign-up" logo, which will be presented to Customer (the "Image"), on an appropriate page of the Customer’s internet site, located at a URL to be provided by the Customer, with a hyperlink to Blackboard’s portal site (the "Link"). The Customer may not use any other trademark or service mark in connection with the Image without the prior written approval of Blackboard. The Link may not be used in any manner to provide a user with access to the portal via any framing, layering or other techniques now known or hereafter developed that permit display of the portal with any materials posted by Customer or any party other than Blackboard. Customer may not allow the Image to be linked to any other web site. The Customer may not use the Image in any manner not permitted hereunder, modify the Image, or copy, or create a derivative work from, the "look and feel" of the Image. Blackboard will have the right to review all uses of the Image for quality control purposes and proper compliance with guidelines, as they may be modified from time to time. The Image and the goodwill associated therewith are valuable properties belonging to Blackboard and all rights thereto are and shall remain Blackboard’s sole and exclusive property. Blackboard reserves the right to modify permission to use the Image and/or the Link at any time.
Portal Access. During the Program you will be provided with access to a web portal allowing you to remotely monitor Equipment sensor data. The portal will be provided by a third party; prior to accessing the portal, you may need to acknowledge such third party’s standard terms of use and privacy notice. To properly assess the Program, HSB needs to monitor your portal and assess the sensor data collected by the Equipment at your location. By signing below you give permission to such third party to provide HSB and Church Mutual (i) access to your portal and (ii) the data collected by the Equipment at your location. HSB and Church Mutual may use all information they collect or learn during the Program, including any such data, as HSB and Church Mutual see fit so long as any identifying information is removed prior to such data being shared with any third party.

Portal Access. Customer will have access to the Portal for reporting analytics and full visibility to their network with the option to change their Service configuration. CenturyLink will be able to perform ongoing configuration changes, and monitoring. Requests for configuration changes must be submitted by calling the MECM operations center. CenturyLink will exclusively maintain global administrative access to the Service at all times. CenturyLink will not be responsible for outages or security incidents that occur due to Customer changes or configuration.
Portal Access. The SL Portal, information regarding your use of the SoftLayer Services, Account information, and support are provided and processed on-line and may be presented only in English. Even if presented during the order process, the terms of SoftLayer’s agreements will not apply unless specifically set forth in this Service Description.
