Physician. Only a duly qualified physician who is legally licensed to practice medicine.
Physician. Shall be defined as a legally licensed medical doctor, Dentist, surgeon, chiropractor, osteopath, podiatrist (chiropodist), optometrist, or clinical Psychologist acting within the scope of his or her license. A Physician shall not include a medical resident, intern, fellow, Physician's assistant, social worker or master prepared therapist.
Physician. As used in this agreement, a health care provider employed by PPHC to provide Member Services to Member.
Physician. The term “Physician” means the individually licensed professionals who are shareholders of Medical Practice, or are employed or otherwise retained by or associated with Medical Practice by contract.

Physician. You understand and acknowledge that your Physician may not be able to accept new members due to patient capacity limits. If your Physician is no longer available to provide Program Services, either Cypress (functioning in an administrative supporting role for your Physician) or your Physician will notify you of such unavailability and will refund any unearned annual fees you have paid if you so desire, as explained in paragraph five below.
Physician. You understand and acknowledge that if you terminate your Program Services arrangement with your Physician, you may be unable to rejoin due to patient capacity limits. If for any reason your Physician is no longer available to provide Program Services, either Cypress (functioning in an administrative supporting role for your Physician) or your Physician will notify you of such unavailability and will refund any unearned Annual Fees you have paid if you so desire, as explained in paragraph five (5) below.
Physician. The term "Physician" shall mean each individually licensed professional who is employed or otherwise retained by or associated with Medical Group, each of whom shall meet at all times the qualifications described in Article V.
Physician. A licensed practitioner of the healing arts acting within the scope of his or her license to the extent provided by the laws of the jurisdictions in which medical treatment is provided. Physician does not include you, an immediate family member, your employer, business partner, or the policyholder.
Physician. A person who holds the degree of doctor of medicine or doctor of osteopathy or its equivalent and who holds a valid unlimited license to practice medicine or osteopathic medicine in the state in which he practices.
