Ongoing Assessment. 4.3.1 The pharmacy contractor should plan to review all patient’s assessment on at least annual basis (sooner may be required if there are a change in patient circumstances). The review month should be annotated on the register of compliance aid patients contained within the Pharmacy Claim Workbook ‘Compliance Aid’ tab. 4.3.2 The pharmacy contractor must complete the ‘Annual review of suitability for pharmacy supplied compliance aid’ form (Appendix 2) in full for all patients at least annually. This should be completed with the patient however where there is diminished capacity it can be a suitable representative e.g. family member or carer. 4.3.3 Where the outcome confirms patients’ continued suitability the patient should continue with the compliance aid. 4.3.4 Where the assessment outcome highlights a change in circumstances which demonstrate that a compliance aid is no longer suitable the pharmacy contractor should discuss this with the patients GP. No patient should have their compliance aid provision stopped without prior discussion and agreement with their GP.

Ongoing Assessment. 6.1 As part of its Management Services, the Contractor will periodically assess the performance of the Program Client’s potential clients, and exposure on the Internet. Relevant factors to be considered when revising the Materials may include marketing, product or service promotion, and sale of products or services. 6.2 The Contractor will inform the Client of the outcome of its assessment, and prior to implementing any proposed updating of the Materials, will obtain approval from the Client.

Ongoing Assessment a re-evaluation of an Enrollee's health status conducted in accordance with Section 2.4.A.11 of the Contract. An Ongoing Assessment for an Enrollee identified as having Complex Care Needs must be conducted in person by a Registered Nurse. EOHHS may prescribe the Ongoing Assessment tool. Opt-In Enrollment – enrollment in a SCO plan initiated by an Eligible Individual. Opt Out – a process by which an Eligible Individual or his/her Authorized Representative chooses not to be enrolled with the Contractor via Passive Enrollment. An Eligible Individual may Opt Out at any time before the effective date of his or her Passive Enrollment.

Ongoing Assessment. Once a customer is assigned a Case Manager, he/she will also receive additional objective assessment of their barriers and strengths in developing an Individual Employment Plan (IEP) to identify program goals. Program Staff will work in direct collaboration with the OEWD Adult and Dislocated Worker Program Coordinator to identify assessment tools (such as the Career Readiness Certification assessed through WorkKeys) as additional and/or viable options in conducting assessments. When a customer’s interest is unidentified, the Talent Development Case Manager will recommend and provide an interest inventory.

Ongoing Assessment a re-evaluation of an Enrollee's health status conducted in accordance with Subsection 2.4(A)(11) of the Contract. Outreach - marketing, including the use of promotional materials, produced in any medium, targeted to Potential Enrollees to promote the Contractor’s program and the use of notification forms and materials to communicate with current Enrollees.

Ongoing Assessment. The performance of suppliers is regularly monitored using key performance indicators (quality, delivery times, responsiveness). Performance reports are shared quarterly by Kinova.

Ongoing Assessment a. Completion of annual reassessment utilizing the required assessment tools identified for the specific waiver and/or state plan programs; b. Timely submission of annual CPOC; c. Follow-up/monitoring; d. Face-to-face visits (in the location, at the frequency and for the purposes specified by the waiver or state plan program requirements); e. Monthly phone contacts; f. Observation of all waiver and state plan services; and g. Monitoring of each direct service provider for each identified waiver and state plan service. The Agency will, at a minimum: a. Assure that services are delivered as identified in the CPOC; assure that services are identified through the OAAS Planning Framework, based on the needs of the participant and not driven by participant, family or provider requests that are not supported by the assessments; b. Identify and address the changing needs of the participant through the reassessment and revision process; c. Assure that services provided are those identified in the CPOC; that the direct service provider’s Service Implementation Plan contains specific timelines and instructions on completing the task(s); that timelines and instructions are followed; progress on Service Implementation Plan activities are tracked on a quarterly basis; and Service Implementation Plan is reviewed internally by the direct service provider; d. Have procedures to identify, address and report problems with provision of services in accordance with OAAS policy and procedures; e. Assure completion of contact and follow-up requirements; f. Identify and differentiate minimum requirement needs for contact and follow-up vs. participant’s needs; g. Identify and address the participant’s need for more intensive support coordination such as more frequent contacts or follow-up that exceed the minimum and/or previously identified requirements; and h. Utilize the CPOC service balance report for the appropriate population and/or service.
