Needs and Services Plan. 16.2.1 The NSP shall be developed by CONTRACTOR in partnership with all of the Xxxxxx Youth’s/NMD’s treatment providers, including CONTRACTOR’s Social Worker and Xxxxxx Youth’s/NMD’s family as appropriate, within the first thirty (30) days of placement.

A progress report identifying the Youth’s strengths and 12 progress in stepping down to a lower level of care shall be prepared by CONTRACTOR with 13 signatures, and submitted to the Youth’s County SW/PO every three (3) months thereafter. The 14 plan shall be based on information, including, but not limited to, the following:
