Local Service Request. Orders for resale of services will be placed utilizing standard LSR forms. GTE will continue to participate in industry forums for developing service order/disconnect order formats and will incorporate appropriate industry standards. Complete and accurate forms (containing the requisite end-user information as described in the Guide) must be provided by NEN before a request can be processed.

VERIZON cannot provide an AIT of service from another CLEC selling VERIZON’s Services to SPRINT.
