INTRODUCTION/SERVICES. A. Contractor shall provide services for California Department of Veterans Affairs, (CalVet). Contractor shall provide services at the Veterans Home (Home) located at 000 Xxxxxxxxxx Xxxxx, Xxxxxxxxxx, XX 00000. Contractor shall provide all labor, staff, transportation, license, permits and every other item of expense necessary, unless otherwise stated, to provide used oil and antifreeze pick-up service, parts washer service, oily rag and oil filter drum removal service. Services shall be provided at the Home, upon request and in compliance with terms of this Agreement.
INTRODUCTION/SERVICES. A. The Contractor agrees to provide services to California Department of Veterans Affairs (CalVet) at the Yountville Veterans Home (Home) located at 000 Xxxxxxxxxx Xxxxx, Xxxxxxxxxx, XX 00000. Contractor shall provide all labor, materials, transportation, license, permits and every other item of expense necessary, unless otherwise stated, to provide pump and motor preventative maintenance to include testing, replacement, scheduled and emergency repair services. Services shall be provided at the Home as scheduled or upon request of the Contract Manager or designee and in accordance with terms of this Agreement.
INTRODUCTION/SERVICES. Genworth will use commercially reasonable efforts to identify corporate and other institutional investors that it believes are appropriate for, and may be interested in the institutional asset management services offered by, GEAM. Upon identifying any such potential clients, Genworth will notify GEAM promptly in writing of the name and address of any such potential clients. Genworth also will use commercially reasonable efforts to assist GEAM in establishing relationships with potential clients identified by Genworth.
INTRODUCTION/SERVICES. A. Consultant shall provide services, as specified, to California Department of Veterans Affairs, (CalVet) at the California Veterans Home of Yountville, California. Consultant shall provide all labor, materials, staff, transportation, license, permits and every other item of expense necessary, unless otherwise stated, to provide Comprehensive Assessment Services. Services shall be provided at the California Veterans Home of Yountville, California in compliance with terms of this Agreement.
INTRODUCTION/SERVICES. A. This is a Statewide Agreement in which the Contractor shall provide Prosthetic and Orthotic services, on an as-needed basis, to patients/youth referred for such medical services from CDCR/CCHCS. Contractor shall provide all labor, materials, staff, transportation, licenses, permits and every other item of expense necessary, unless otherwise stated. Services shall be provided on-site upon request and under the direction of the institution Chief Executive Officer or Chief Medical Executive (CEO/CME) or designee, or the Division of Juvenile Justice Facility (facility) Chief Medical Officer (CMO) or designee. For purposes of this Agreement, CDCR/CCHCS designee must be a civil service employee. The institution CEO/CME or designee, and facility CMO or designee retains professional and administrative responsibility for services rendered.
INTRODUCTION/SERVICES. The Contractor agrees to provide staffing services incidental to the operation of five (5) inmate camp facilities and inmate fire suppression crews for the purpose of fire suppression, fire prevention, pre-suppression, reforestation, afforestation, and emergency and general work projects, as mutually agreed to by the California Department of Corrections & Rehabilitation (CDCR) and the Consolidated Fire Protection District of Los Angeles County (LAC Fire Department). The LAC Fire Department and the CDCR reserve the right to make adjustments to the staffing commensurate with the State’s funding for the program. If staffing adjustments and/or camp closures become a necessity, either party (LAC Fire Department or the CDCR) shall notify the other in writing 60 days prior to the effective date of the change and the CDCR shall reduce the camp inmate population accordingly, commensurate with the number of fire crews in each camp (i.e., four fire crews equals an 80-inmate camp, five fire crews equals a 100-inmate camp, etc.). Both parties agree that if fire crews are reduced, the minimum inmate population shall be no less than 80 inmates per camp. The reduced staffing, number of inmates and crews will be reflected on revised provisions of Exhibit A, Scope of Work, and Exhibit B-1, Projected Budget, where applicable. INTRODUCTION/SERVICES. 1.1 This Agreement is the result of a Request for Proposal (the “RFP”) initiated by EDC on [DATE OF RFP] for the pre- qualification of a list of suppliers (“Qualified Supplier Roster”). Pursuant to the evaluation criteria and qualification process set out in the RFP (the “Qualification Process & Requirements”), the Supplier was selected to join the Qualified Supplier Roster and invited to enter into this Agreement with EDC.
INTRODUCTION/SERVICES. A. This is a Statewide Agreement for the purpose of obtaining and providing on-site Oral and Maxillofacial Surgeon (OMFS) Services for referred patients/youths for whom California Department of Corrections and Rehabilitation (CDCR)/California Correctional Health Care Services (CCHCS) has the responsibility to provide such services. OMFS services include, but are not limited to, the Procedures listed or described in Exhibit B-1, Bid Rate Sheet, and/or B-2, CDCR/CCHCS Standardized Fees to this Agreement.

Contractor shall furnish all labor, licenses, permits, supplies, materials, insurance, packaging, tools, vehicles, placards, labels/markings, and equipment (including personal protective equipment and safety equipment) required to provide services for collection, disposal, transportation and disposal of Hazardous, Resource Conservation and Recovery Act (RCRA) and Universal wastes (excluding radioactive or biohazard waste), and other items of expense necessary, unless otherwise stated. All work by the Contractor will be under the direction of the CalVet Contract Manager or his/her designee, in accordance with all federal, state, local laws, and regulations.
