Execution and Amendments. This Acknowledgement may be executed in multiple counterparts, and will have the same legal force and effect as if the Parties had executed it as a single document. The Parties may sign in writing or by electronic signature. An electronic signature, facsimile copy, or computer image of a signature, will be treated, and will have the same effect as an original signature, and will have the same effect, as an original signed copy of this document. This Acknowledgement may be amended or modified only by a written instrument signed by authorized representatives of both Parties.

Execution and Amendments. This Agreement shall be executed by ▇▇▇▇▇▇▇▇’s signing the Network Registration (Provider Direct) Form. This Agreement may be amended from time to time solely by UMC. Each time Provider receives disbursement from UMC, Provider shall be required to review and agree to the terms and conditions of this Agreement existing as of the date Provider signs a Disbursement Form. Therefore, by signing the Disbursement Form, Provider agrees to the latest version of the terms and conditions of this Agreement, and the changes, if any, made subsequent to Provider’s signing of Network Registration (Provider Direct) Form. To the extent permitted by law, if any signature to this Agreement or any amendment hereto is delivered by facsimile transmission or by e-mail delivery of a “.pdf” format data file, such signature shall create a valid and binding obligation of the party executing (or on whose behalf such signature is executed) with the same force and effect as if such facsimile or “.pdf” signature page were an original thereof.
