CONTRACTED PARTIES. A. Placer County Adult System of Care, hereafter referred to as Contractor, agrees to provide Forensic Conditional Release Program (CONREP) services, (as defined in Section 6) to the Department of State Hospitals (DSH) pursuant to the terms and conditions of the Agreement.
CONTRACTED PARTIES. The PARTIES acknowledge that the direct or indirect costs described above may undergo some fluctuation after this AGREEMENT has been accepted by the CONTRACTED PARTIES and while it is being performed, due to foreseeable events such as the need to substitute materials or use of alternative means of production, project adjustments, the CONTRACTED PARTIES undertaking to bear the risk for these fluctuations and the ensuing added costs, observing the provisions in item 8.6. So that no doubts may arise as to the foreseeability of certain events, the following are hereby classified as foreseeable, in addition to the aforementioned items:
CONTRACTED PARTIES. A. Correctional Healthcare Associates, hereafter referred to as Contractor, agrees to provide temporary relief physician and non-physician clinical services through qualified staff to Department of State Hospitals (DSH) pursuant to the terms and conditions of this emergency Agreement. The existing services provided under Agreement 16- 09001-000 began February 1, 2017 and remains in full force and effect. This emergency Agreement does not alter or amend the terms and conditions of the existing Agreement; however, this emergency Agreement expands and supplements the existing services.
CONTRACTED PARTIES. This contract is between Synergy Direct Solution, LLC and the individual/company listed above.
CONTRACTED PARTIES. A. Merced County and/or their authorized designee, hereafter referred to as Contractor, agrees to provide services (as defined in Section 6) to the Department of State Hospitals (DSH) pursuant to the terms and conditions of this Agreement.

The Department of State Hospitals is hereafter referred to as “DSH”.
CONTRACTED PARTIES. The County of Tuolumne and/or their authorized designee, hereafter referred to as “Contractor,” agrees to provide services as defined herein pursuant to the terms and conditions of this Agreement.
CONTRACTED PARTIES. A. This Agreement is made and entered into by and between, the Department of State Hospitals (DSH) and Orange County, hereafter referred to as Contractor, to provide DSH with relevant inmate medical records, including mental health and behavioral health records, and coordinate telehealth video interviews of defendants who have been committed to DSH as Incompetent to Stand Trial (IST) on a felony charge, and are awaiting admission to a DSH facility. Services are to be provided in accordance with Welfare and Institutions Code (WIC) section 4335.2 and are identified as IST Re-Evaluation services herein. IST Re-Evaluation Services performed by DSH will be for the purposes of re-evaluating competency status, psychotropic medication status, and assessment for placement in an alternative community based programs including but not limited to mental health diversion or community based restoration.
CONTRACTED PARTIES. A. The County of Santa ▇▇▇▇▇▇▇ and/or their authorized designee, hereafter referred to as “Contractor,” agrees to provide services (as defined in Section 4) pursuant to the terms and conditions of this Agreement.
