CONTRACTED PARTIES. A. Correctional Healthcare Associates, hereafter referred to as Contractor, agrees to provide temporary relief physician and non-physician clinical services through qualified staff to Department of State Hospitals (DSH) pursuant to the terms and conditions of this emergency Agreement. The existing services provided under Agreement 16- 09001-000 began February 1, 2017 and remains in full force and effect. This emergency Agreement does not alter or amend the terms and conditions of the existing Agreement; however, this emergency Agreement expands and supplements the existing services.
CONTRACTED PARTIES. 6.2.1 The PARTIES acknowledge that the direct or indirect costs described above may undergo some fluctuation after this AGREEMENT has been accepted by the CONTRACTED PARTIES and while it is being performed, due to foreseeable events such as the need to substitute materials or use of alternative means of production, project adjustments, the CONTRACTED PARTIES undertaking to bear the risk for these fluctuations and the ensuing added costs, observing the provisions in item 8.6. So that no doubts may arise as to the foreseeability of certain events, the following are hereby classified as foreseeable, in addition to the aforementioned items:

IST Re-Evaluation Services performed by DSH will be for the purposes of re-evaluating competency status, psychotropic medication status, and assessment for placement in an alternative community based programs including but not limited to mental health diversion or community based restoration.

LF Staffing Services, Inc., hereafter referred to as Contractor, agrees to provide temporary relief food, custodial and skilled trade services through qualified staff to the Department of State Hospitals (DSH) pursuant to the terms and conditions of this emergency contract (Emergency Agreement).

CONTRACTED PARTIES. The County of Tuolumne and/or their authorized designee, hereafter referred to as “Contractor,” agrees to provide services as defined herein pursuant to the terms and conditions of this Agreement.
CONTRACTED PARTIES. This contract (Environmental Management Plan-EMP) is a working agreement between the contract parties of Hilton Parish Council (HPC) and the supplier of contracted services, referred to here as ‘the Contractor’. The EMP shall begin on 1st April 2022 and will run for a period of 36 months thereafter. Following the start of the EMP, the Contractor will be required to give three months’ notice of termination. Hilton Parish Council may terminate the contract with one months’ notice. Amendments to the schedule will be agreed as required between the Parish Council and the Contractor. During this period the Contractor/s will provide services as described below.
