BENEFITS WAIVER. If CONSULTANT is unincorporated, CONSULTANT acknowledges and agrees that CONSULTANT is not entitled to receive the following benefits and/or compensation from REGIONAL SAN: medical, dental, vision and retirement benefits, life and disability insurance, sick leave, bereavement leave, jury duty leave, parental leave, or any other similar benefits or compensation otherwise provided to permanent civil service employees pursuant to the County Charter, the County Code, the Civil Service Rule, the Sacramento County EmployeesRetirement System and/or any and all memoranda of understanding between REGIONAL SAN and its employee organizations. Should CONSULTANT or any employee or agent of CONSULTANT seek to obtain such benefits from REGIONAL SAN, CONSULTANT hereby agrees to indemnify, defend, and hold REGIONAL SAN harmless from any and all claims, including reasonable attorneys’ fees, that may be made against REGIONAL SAN for such benefits.

BENEFITS WAIVER. Effective fall 2010, for those faculty members who waive health care benefits and were hired prior to 2004, the annual payment amount shall be $3800. For those faculty members who waive health care benefits and were hired in 2004 and thereafter, the annual payment amount shall be $2600. Faculty members who return to College-provided health care benefits may select the plan or the equivalent in which they were enrolled at the time they elected the waiver or any other plan currently open to new employees.
BENEFITS WAIVER. 1. I am either an employee of a company (a “Supplier”) that has contracted with Verizon Sourcing LLC (“Verizon”) to provide services, a contractor for Supplier, or an employee of a subcontractor or staffing firm providing services to Supplier in connection with work performed by the Supplier pursuant to Supplier’s contract with Verizon.
BENEFITS WAIVER a. Voluntary Buyback- To be eligible for the voluntary buyback, an employee shall provide written documentation that he/she maintains health coverage under another plan or subscribership and therefore will not need the medical insurance coverage provided by the District. Such proof must be presented at the time he/she applies for the waiver. An employee who is eligible for health benefits provided and paid for by the District may voluntarily waive insurance coverage. In exchange for this annual waiver, the District shall pay the eligible employee $2500 per year. Payment by the District of the $2500 buyback shall be paid each July 15th of the year following the annual waiver. Employees receiving the voluntary buyback payment may decide whether to receive the payment by payroll check, a deposit into the employee’s 403b plan or a deposit into a 125 Plan established by the District. New hires shall be eligible for proration during their first year of coverage, based upon their date of eligibility. Should a life-changing event occur (as defined by Blue Cross/Blue Shield) that would require the District to reinstate coverage for an employee who selected the voluntary buyback, the District will pay him or her a monthly prorated portion of the $2500. The pro-rated payment shall be paid no later than July 15th of the year following the buyback.
