DENTAL AMENDATORY RIDER B PROSTHODONTICS The following prosthetic services are provided under Dental Amendatory Rider B: ♦ Denture, full and partial ♦ Bridges, fixed and removable ♦ Addition of teeth to partial dentures to replace extracted teeth ♦ Occlusal night guards-(new for 07-01-2014) **Occlusal Night Guards will be paid at the 80% percent of Anthem’s Reasonable and Customary charge and member would be responsible for the coinsurance when using a participating dentist.
