Program Performance Measures. 1. Contractor shall administer the same district and state-required assessments in the same manner and timeframe and follow, at a minimum, the same promotion and graduation requirements as all MPS schools, as described in the MPS Administrative Policy 7.37, Graduation Requirements, attached hereto and incorporated herein by reference as Appendix D, or as approved in writing by the Director of Contracted School Services. Contractor explicitly agrees to comply with the assessment and accountability provisions of P.L. 114-95, Every Student Succeeds Act of 2015 (ESSA) as they are made applicable to MPS and Contractor.

Program Performance Measures. Performance criteria shall be used to assess the level of performance of Contractor and considered by County when determining future funding. In order to be considered in compliance with the performance criteria, Contractor must submit to County a request for reimbursement which demonstrates that Contractor has expended funds and met their proposed accomplishment goals at the required levels, unless exempted in writing by the Director. Failure to achieve performance may cause any remaining balance in this Contract to be reclaimed by County and may negatively affect future funding to Contractor. The table below itemizes the performance criteria the Contract will be measured against. County also may monitor performance using the available HMIS reports pertaining to the Program. For the life of the Contract, Contractor shall meet or exceed the following performance metrics: Navigation Center Performance Metrics 134 Minimum number of unduplicated persons experiencing homelessness served 38 Minimum number of unduplicated households experiencing homelessness served 134 Of those served, minimum number of unduplicated persons experiencing unsheltered homelessness 38 Of those served, minimum number of unduplicated households experiencing unsheltered homelessness 120 Average length of stay (in days) for participants in Program 10% Minimum % of participants will increase earned income at Program exit 15% Minimum % of participants will increase cash/non-cash benefits at Program exit 40% Minimum % of participants will exit the Program to permanent housing 40% Minimum of % of households will exit the Program to permanent housing 100% Of those who exit to permanent housing, % of participants who remain in permanent housing for three (3) months 80% Of those who exit to permanent housing, minimum % of participants that return County of Orange, Health Care Agency Contract MA-042-20010823 to homelessness within six (6) months of Program exit

Program Performance Measures. (a) CONTRACTOR will track, at a minimum, the performance measures identified below and detailed in program instructions prepared by COUNTY and incorporated into this Contract by reference. CONTRACTOR will document and report this data to COUNTY as described in Section C .3. “Reporting Requirements”. CONTRACTOR agrees to participate with the COUNTY in evaluation of contracted project/service outcomes or performance, and to make available all information required by such evaluation process. This includes providing COUNTY with data necessary to verify client counts, service provision, and outcome measures.

  • Ongoing Performance Measures The Department intends to use performance-reporting tools in order to measure the performance of Contractor(s). These tools will include the Contractor Performance Survey (Exhibit H), to be completed by Customers on a quarterly basis. Such measures will allow the Department to better track Vendor performance through the term of the Contract(s) and ensure that Contractor(s) consistently provide quality services to the State and its Customers. The Department reserves the right to modify the Contractor Performance Survey document and introduce additional performance-reporting tools as they are developed, including online tools (e.g. tools within MFMP or on the Department's website).

  • Performance Measures The System Agency will monitor the Grantee’s performance of the requirements in Attachment A and compliance with the Contract’s terms and conditions.
