Community Care definition

Community Care means the Community Care program which provides Community Care services for people under 65 years of age and for Aboriginal and Xxxxxx Xxxxxx Islander people under 50 years of age;
Community Care means Churches of Christ Community Care.
Community Care means child- and family-focused services and supports provided to families re- ferred from the department. Services shall be geared toward keeping the children in the family safe from abuse and neglect; keeping the family intact; preventing the need for further intervention by the department, including removal of the child from the home; and building ongoing linkages to community-based resources that improve the safety, health, stability, and well-being of families served.

  • A Community Care Worker would not normally live at the client’s residence for periods in excess of 48 hours.
