Personal Effects. The Employer shall replace or repair any employee's dentures and/or prescription spectacles which are damaged or destroyed in the course of the employee's ordinary duties, other than through the employee's own negligence up to a maximum of $150.00 for each set of spectacles and/or dentures. Provided that the Employer may require the employee to furnish a statutory declaration setting out the circumstances of the damage or destruction and supporting evidence of the value of the item damaged or destroyed shall be furnished by the employee.
