Field House. The field house is open to members and paid guests during CERC hours. YOUTH UNDER THE AGE OF 8 REQUIRE DIRECT SUPERVISION BY A PARENT, LEGAL GUARDIAN OR AUTHORIZED ADULT WHILE IN THE CERC. Youth over the age of 8 will be allowed to play independently as long as an adult is in the CERC; however, if they cannot manage their behavior and follow rules, those privileges will be revoked. Hanging on or grabbing basketball rims/nets and volleyball nets is prohibited at all times. All running, tagging, ball throwing is to be confined to the field house. The four-lane track may be used for both walking and jogging. Walkers are asked to please use the inside lanes and joggers use the outside lanes. Strollers with clean rubber wheels are allowed on the track. You may not organize your own informal or formal sports leagues, tournaments, or programs, including but not limited to basketball, tennis, and volleyball. THE MEMBER AGREES to abide by all facility policies and procedures and CERC rules. Because exercise can be strenuous and subject to risk of serious injury, ISD 717 urges you to consult a physician before beginning any exercise program. You, the member, agree that if you engage in any physical exercise or activity, or use any of the fitness center equipment in the facility, you do so entirely at your own risk. This includes without limitation, your use of the locker rooms, parking area, sidewalk area, or any equipment in the fitness center, and your participation in any activity, class, program, or instruction. You agree that you are voluntarily participating in activities and the use of the facility and assume all risks of injury, illness, damage or loss by theft of any personal property. You release and discharge ISD 717/Jordan Community Education and Recreation Center and all affiliates, employees, agents, representatives, successors, or assigns from any and all claims of action. The agreement, waiver, and release of liability include, without limitation, all injuries to you, which may occur, regardless of negligence: as a result of (A.) your use of the exercise equipment, (B) the sudden and unforeseen malfunctioning of any equipment, (C) our instruction or supervision and (D) your slipping and or falling while in the facility, or on the ISD 717 premises. Field House. The approximately 98,000 square–foot field house situated on the Land presently known as the “Healthy Zone Fieldhouse” and presently housing the Team’s indoor practice facility.
Field House. What to wear: Solid white T-shirt tucked in to plain black or navy Nike style shorts - Hair completely out of face in a ponytail, with a solid plain white bow. - No jewelry or gum. Only girls trying out for cheerleader, sponsors, judges, and senior cheerleaders will be allowed in the Field House and SLC. No parents or friends will be allowed in the FH or SLC. Once each cheerleader has completed the tryout process, she will need to leave the Field House. There will be mats for tryouts! Each grade level will be in groups of three for try-outs.   • Chiropractic Services This plan covers chiropractic visits up to the benefit limit shown in the Summary of Medical Benefits. The benefit limit applies to any visit for the purposes of chiropractic treatment or diagnosis.

  • Cable Television, Telephone & Internet Services Long Distance calling may be done only through the use of a prepaid phone card or by charges made to a third party number if using a provided, in room telephone. Neither the Institution nor the Manager guarantees the availability of telephone service or cable television services. If the Resident wants additional cable television, telephone or internet service above and beyond any that may be provided as “standard” in the Residence, the Resident must submit full details to and request and obtain the prior written approval of the Manager and Institution. With respect to Internet Services, the Institution may at its discretion only allow either the standard provided service or the approved alternate service and not both at the same time (Residents will need to complete the Institution’s standard forms issued by their IT department). Cutting of wiring, boring of holes, the use of wireless routers, routers, or switches are not permitted. Any unauthorized services or equipment may be removed by the Manager, at the Resident’s expense, without notice or liability. All Residents are subject to the Institution’s and/or Service Provider’s current Internet, cable television and telephone enrolment and usage policies.

  • Diagnostic Services Procedures ordered by a recognized Provider because of specific symptoms to diagnose a specific condition or disease. Some examples include, but are not limited to:

  • New York State Statewide Financial System New York State Statewide Financial System (SFS) went live for NYS agencies in April 2012. Future SFS procurement functionality envisions the ability to fully host Contract catalogs, to integrate Contractor-hosted punch-out catalogs, and/or to submit and process invoices electronically. OGS reserves the right to integrate any or all of these future catalog functions with a Contractor during the contract period, and by submittal of a Vendor Submission, a Vendor agrees to coordinate with SFS, OGS and/or a third party host, for integration, if OGS exercises its right to do so. No costs or expenses associated with providing information and integration shall be charged to NYS. Technical Requirements for the data elements, such as data types, maximum field lengths, and cXML element names shall be provided by SFS, OGS and/or a third party host during integration.   • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.

  • Beta Services From time to time, We may invite You to try Beta Services at no charge. You may accept or decline any such trial in Your sole discretion. Beta Services will be clearly designated as beta, pilot, limited release, developer preview, non-production, evaluation or by a description of similar import. Beta Services are for evaluation purposes and not for production use, are not considered “Services” under this Agreement, are not supported, and may be subject to additional terms. Unless otherwise stated, any Beta Services trial period will expire upon the earlier of one year from the trial start date or the date that a version of the Beta Services becomes generally available. We may discontinue Beta Services at any time in Our sole discretion and may never make them generally available. We will have no liability for any harm or damage arising out of or in connection with a Beta Service.
