Eligibility Form. If the unit member and the evaluator mutually consent that the unit member is eligible for the five-year evaluation cycle, by no later than September 10, both the unit member and the site administrator/evaluator will sign the eligibility form attached to this Agreement as Appendix J-6. The signed and completed eligibility form shall be placed in the unit member’s District personnel file.
