Request for Records. In the event that any requests or demands are made for the inspection of the Shareholder records of the Customer, other than request for records of Shareholders pursuant to standard subpoenas from state or federal government authorities (e.g., in divorce and criminal actions), the Transfer Agent will endeavor to notify the Customer and to secure instructions from an authorized officer of the Customer as to such inspection. The Transfer Agent expressly reserves the right, however, to exhibit the Shareholder records to any person whenever it is advised by counsel that it may be held liable for the failure to exhibit the Shareholder records to such person or if required by law or court order.
Request for Records. Within 5 days of entry of a student the AGENCY’s appointed staff will request the educational records (including ESE information) from all previous schools of attendance.
Request for Records. Any request from a Market Partner for copies of invoices, applications, Market Partner Activity Reports, or other records will require a fee of $2.00 per page per copy. This fee covers the expense of mailing and time required to research files and make copies of the records.
Request for Records. Therapy is most effective when a trusting relationship exists between the therapist and the client. Privacy is especially important in securing and maintaining that trust. By signing this agreement, you will be waiving your right to access you or your child’s treatment records except for treatment summaries provided upon request. If I am required to testify, a judge may order us to produce records. A subpoena signed by an attorney will not be enough for us to release your child’s therapy records.
Request for Records. The District and LBCCE shall have the right to receive within ten (10) days of request from each other all documents necessary and relevant to investigating and processing the grievance. The documents shall be those legally available to both parties and the production of which shall not be unduly burdensome to the District or LBCCE.

In the event that any requests or demands are made for the inspection of the Shareholder records of the Customer, other than the request for records of Shareholders pursuant to standard subpoenas from state or federal government authorities (e.g., in divorce and criminal actions) which do not name any of the Funds, Massachusetts Financial Services Company, MFS Fund Distributors, Inc. or MFS Service Center, Inc., the Transfer Agent will promptly notify the Customer and attempt to secure instructions from an authorized officer of the Customer as to such inspection, and the Customer shall promptly (i) provide the Transfer Agent with such instructions or (ii) inform the Transfer Agent that the Customer or the MFS entities referenced above will respond directly to such request or demand. The Transfer Agent expressly reserves the right, however, to exhibit the Shareholder records to any person whenever it is advised by counsel that it may be held liable for the failure to exhibit the Shareholder records to such person or if required by law or court order.
Request for Records. If the ACBN, or any authorized agency or organization requests the health care records, x-rays, or other documents, certificant shall provide copies of these records within a time frame as determined by the ACBN, usually fourteen (14) days. Copies of records shall be provided at Certificants sole expense. Failure of Provider to supply requested records may result in termination of this Agreement.
Request for Records. The Province, the Auditor General of British Columbia and any independent auditor identified by the Province (in each case, the “Inspecting Party”), will be entitled to request copies of, and access to, the Recipient’s records referred to in section 14.1 from time to time during the Audit Period for audit, verification, or reporting purposes, and may provide all such records and information to its third party advisors or the Province.

Requests for printed genealogies will require a fee of three [3] cents per line and with a minimum cost of $12 plus postage. Request for Records. A. The School District agrees to furnish to the Association official public financial records and other such records necessary to the resolution of grievances and the collective bargaining process following specific written requests setting forth the reasons therefore.
