Covered Service. A health care service to which an Enrollee is entitled under the terms of a Health Benefit Plan.
Covered Service. All health care services and benefits the Contractor must arrange to provide to Members, including all services required by the Contract and state and federal law, and all other services negotiated by the Parties.
Covered Service. A health care service or product for which a Customer is entitled to receive coverage from a Payer, pursuant to the terms of the Customer’s Benefit Plan with that Payer.
Covered Service a Service or supply, including vision care services, specified in this Agreement which is eligible for payment when rendered by a Provider or Supplier.
Covered Service a service or supply specified in the Agreement and summarized in Section CS - Description Of Covered Services for which benefits will be provided. CUSTODIAL CARE (DOMICILIARY CARE) – care provided primarily for maintenance of the patient or care which is designed essentially to assist the patient in meeting his activities of daily living and which is not primarily provided for its therapeutic value in the treatment of an illness, disease, bodily injury, or condition. Custodial Care includes, but is not limited to, help in walking, bathing, dressing, feeding, preparation of special diets and supervision of self-administration of medications which do not require the technical skills or professional training of medical or nursing personnel in order to be performed safely and effectively. DEDUCTIBLE – The amount of the Allowable Amount that must be incurred by a Member each Agreement Year before benefits are covered under this Subscriber Agreement. Deductibles, if any, are identified in Section SC - Schedule Of Cost Sharing. DEPENDENT –
Covered Service. In accordance with Article Eleven of ServiceMaster's Certificate of Incorporation (the "Charter"), the services provided and to be provided by you under this Agreement, shall be deemed to be a "Covered Service" within the meaning of subsection 11.1.1 of the Charter and, to the extent any services are not covered by any of subsection 11.1.1(a), (b) or (c) of the Charter, this paragraph and the signature of the Chief Executive Officer on this Agreement shall constitute a designation in writing as required by Article Eleven of the Charter.
Covered Service a service or supply specified in this Contract for which Benefits will be provided when rendered by a Provider.
Covered Service a Service or product specified in this Agreement which is eligible for payment.
Covered Service. A Service or Supply furnished by a provider to a Covered Person for which such Covered Person’s Policy provides coverage. Defective Services or Supplies. Services or Supplies lacking in some particular essential to the completion of an intended result or provided in such a manner that such intended result for the Covered Person could not reasonably be expected to occur.
Covered Service. A medical service or supply that qualifies for payment under terms set forth by CMS and the applicable Plan Document and that meets the Payment Determination Criteria set forth in Section 4.2 of this Agreement or that is a preventive service that is specifically described as covered in the Plan Document.
