Billing and Payments. Transmission Provider shall bill the Interconnection Customer for the Costs associated with the facilities contemplated by this ISA, estimates of which are set forth in the Specifications to this ISA, and the Interconnection Customer shall pay such Costs, in accordance with Section 11 of Appendix 2 to this ISA and the applicable Interconnection Construction Service Agreement. Upon receipt of each of Interconnection Customer’s payments of such bills, Transmission Provider shall reimburse the applicable Interconnected Transmission Owner. Billing and Payments. Your health insurance may cover all or part of the fees and I will work with you to facilitate the exchange of information with your insurance company for payment. However, you are ultimately responsible for all fees incurred. You should contact your health insurance company or consult with me for additional information regarding payment arrangements. Each client is responsible for payment for services at each session. - 50-55 minute individual or couples counseling session: $290* - Late Cancel (within 48 hours for any reason) or No Show: $290* - 90-minute group therapy session: $80* * At the beginning of each year I make an inflation adjustment to keep up with the real amount of our fee agreement. Please note there will be $35 service fee for insufficient funds or payments that are not made according to our payment agreement. This $35 fee is in addition to the original payment due to me. I also charge $290 per hour for other professional services you may need, though I will break down the hourly cost if I work for periods of less than one hour. Other services include telephone conversations lasting longer than 5 minutes, consultation services, and the time spent performing any other service you may request of me. If you become involved in legal proceedings that require my participation, you will be expected to pay for all of my professional time, including preparation and transportation costs and time, even if I am called to testify by another party. Due to the nature of legal involvement, my rate for participation in legal activities is $500 per hour. INSURANCE REIMBURSEMENT In order for us to set realistic treatment goals and priorities, it is important to evaluate what resources you have available to pay for your treatment. If you have a health insurance policy, it will usually provide some coverage for mental health treatment. I will provide you with whatever reasonable assistance I can in helping you receive the benefits to which you are entitled; however, you (not your insurance company) are responsible for full payment of my fees. This includes, but is not limited to, full responsibility for all sessions that exceed any session limits imposed by your insurer. Billing and Payments. You shall pay all fees or charges to your account in accordance with the fees, charges, and billing terms in effect at the time a fee or charge is due and payable. Where Services are offered on a free trial basis, payment may be required after the free trial period ends, and not when you enter your billing details (which may be required prior to the commencement of the free trial period). If auto-renewal is enabled for the Services you have subscribed for, you will be charged automatically in accordance with the term you selected. If, in our judgment, your purchase constitutes a high-risk transaction, we will require you to provide us with a copy of your valid government-issued photo identification, and possibly a copy of a recent bank statement for the credit or debit card used for the purchase. While much can be accomplished in short-term therapy, some patients feel that they need more services after insurance benefits end. Some managed-care plans will not allow me to provide services to you once your benefits end. If this is the case, I will do my best to find another provider who will help you continue your psychotherapy. Your contract with your health insurance company requires that I provide it with information relevant to the services that I provide to you. Information typically requested includes the diagnosis, current symptoms, treatment plan, and progress. Billing and Payments. The Accounts have various billing and payment structures, including varying periodic finance charges and fees. The following is information on the current billing and payment characteristics of the Accounts. Monthly billing statements are sent by the Seller to cardholders at the end of the billing period. Each month, the cardholder must make a minimum payment equal to the greater of (a) $10 plus the outstanding periodic finance charges and (b) the amount by which the then current balance exceeds the authorized credit limit. A balance of $10 or less must be paid in full. The full amount of any unpaid minimum payment required in respect of preceding monthly billing statements, will also be added to the minimum payment. A periodic finance charge is assessed on Accounts at the annual interest rates specified at the time each Account is opened and from time to time thereafter in the monthly billing statements relating to such Account. Periodic finance charges and fees are charged on balances for purchases, cash advances and fees starting from the transaction date until the date the payment is received. No interest is assessed on purchases and fees as long as the total new balance which is owed for the month is paid in full by the payment due date, and for small business credit cards only, the total new balance for the previous month was also paid in full by that month’s statement’s payment due date. The periodic finance charges for any month are calculated on a daily basis by multiplying each daily periodic finance charge bearing closing balance by the equivalent daily rate for the applicable annual rate. The average periodic finance charge on the Accounts relating to cash advances and purchases of goods and services was 18% per annum for the month of June 2016 (excluding introductory interest rates). The Seller may change the periodic finance charge on the various types of Accounts at any time by not less than 30 days’ written notice to the cardholders. The Seller may assess an annual or membership fee that varies depending on the features of the Account. Payments by cardholders to the Servicer on the Accounts are processed and applied first to the minimum payment, then to any remaining balance. If the different amounts that make up the remaining balance are subject to different interest rates, the payments are generally allocated on a pro rata basis. Billing and Payments. Pole Owner shall send invoices to Licensee via regular U.S. Mail at the address specified below, or at such other address as Licensee may designate from time to time. Invoices for rental charges will be sent annually or semi-annually.
