Single Membership: This option provides benefits for Covered Services provided to the Subscriber only.2. Subscriber-Spouse Membership: This option provides benefits for Covered Services provided to the Subscriber and his or her spouse.3. Single Parent Membership: This option provides benefits for Covered Services provided to the Subscriber and his or her Eligible Dependent children, but not to a spouse.
Annual fees: $16.00 for a Family or Newsletter Membership, $14.00 for a Single Membership, $12.00 for a Senior Membership (single or family over 65), and $12.00 for junior members under the age of 18 who are not included in a family.
Any dependent of a Subscriber who has a Single Membership, except as specified in this Contract for newborn or adopted children.

This product is available as a Single Membership, Couple Membership, Family Membership or Sole Parent Membership.
If a person ceases to be an Eligible Dependent, We may provide a comparable Single Membership for that person, provided eligibility and underwriting requirements are met.
