Missed Appointment Fee. We require at least 48-hour notice in order to change any appointments. Those who fail to do so twice within a 6 month period will be asked to pay a Missed Appointment Fee of $75.00 for their exam or visit upon scheduling for the 3rd time. Related to Missed Appointment Fee

  • Restricted Appointment means (a) any Appointment for which the applicable Authorizations have not been obtained, and (b) any Appointment for which the maturity date of the applicable Corporate Trust Contract is on or before June 30, 2022.
