With Full Pay. (i) Critical Illness - Critical illness shall mean a life-threatening illness. For the critical illness or illness requiring admission to hospital intensive care unit of father, mother, husband, wife, son, daughter, brother, sister, parents of spouse and siblings of spouse - not more than five days or until death occurs. A statement will be required if death does not occur, signed by the teacher, signifying to the critical nature of illness, or the illness requiring admission to a hospital intensive care unit and the qualified medical practitioner's name, address and telephone number who may be contacted to verify the statement. The statement shall contain an authorization allowing the doctor to release information relating to the illness giving rise to the leave to the secretary-treasurer of the Board.
