State Contract. Health Plan’s contract with the Department for the purpose of providing and paying for Covered Services to Covered Persons enrolled in the State Program that requires Health Plan and Subcontractor to meet certain performance standards while doing so.
State Contract. All tasks performed under the Agreement shall be performed in accordance with the requirements of the State Contract and LDH issued guides, the applicable provisions of which are incorporated into the Agreement by reference. Nothing in the Agreement relieves United of its responsibility under the State Contract. If any requirement or provision of the Agreement or this Appendix is determined by LDH to conflict with the State Contract, the terms of the State Contract shall control and the terms of the Agreement or this Appendix in conflict with those of the State Contract shall be null and void. All other provisions of the Agreement and this Appendix shall remain in full force and effect.
State Contract. Whenever available, approved vehicles shall be rented in accordance with the intermediate size vehicle listed on the annual contract for rental cars competitively bid by the State of Florida. An explanation for use of rental cars obtained from a source other than the State contract vendor must be justified in writing and approved by the Chief Executive Officer in advance. Use of a rental vehicle larger than an intermediate size described in the State contract, which is required to transport business clients or materials, must be included in the Travel Authorization & Expense Report and approved by the Chief Executive Officer or Department Director in advance.

State Contract. United’s contract(s) with LDH for the purpose of providing and paying for Covered Services to Covered Persons enrolled in the State Program.

The applicable provisions of the State Contract are incorporated into the Agreement by reference. Nothing in the Agreement relieves Health Plan of its legal responsibilities under the applicable State Contract; Health Plan remains fully responsible for meeting all terms and requirements of the State Contract. If any provision of the Agreement or this Appendix is in conflict with provisions of the applicable State Contract, the terms of the State Contract shall control and the terms of the Agreement or this Appendix in conflict with those of the State Contract will be considered waived.
State Contract. A contract between Health Plan and a Department for the purpose of administering and paying for Covered Services to Covered Persons enrolled in the State Program. The State Contract (i.e., YH14-0001; or like successor State Contracts), as amended, includes the AHCCCS solicitation and all attachments, exhibits and amendments thereto, and the proposal and best and final offer accepted by AHCCCS from Health Plan.

All tasks performed under the Agreement shall be performed in accordance with the requirements of the applicable State Contract, as set forth in this Appendix, the Guide to the Senior Care Options Program for MassHealth Providers UHC/PRO.ST.PROG..XXXXXX.00.00.XX UnitedHealthcare Confidential and Proprietary distributed by the Department, and applicable provider manual(s), protocols, policies and procedures that Health Plan or Subcontractor has provided, delivered or made available to Provider. The applicable provisions of the State Contract are incorporated into the Agreement by reference. Nothing in the Agreement relieves Health Plan of its legal responsibilities under the applicable State Contract; Health Plan remains fully responsible for meeting all terms and requirements of the State Contract. If any provision of the Agreement or this Appendix is in conflict with provisions of the applicable State Contract, the terms of the State Contract shall control and the terms of the Agreement or this Appendix in conflict with those of the State Contract will be considered waived.
