State Program. The State of Louisiana’s Healthy Louisiana and related programs where United provides services to Louisiana residents through a contract with the State. For purposes of this Appendix, “State Program” may refer to the State agency(ies) responsible for administering the State Program.
State Program. The Employer’s obligation to implement this Article shall be contingent upon the establishment of the programs described herein by the State of Ohio,
State Program. KanCare, the Medicaid and CHIP program developed and administered by the State of Kansas. For purposes of this Appendix, State Program may refer to the State agency(ies) responsible for administering the applicable State Program.
State Program. The Florida Medicaid program. For purposes of this Appendix, State Program may refer to the State agency(ies) responsible for administering the applicable State Program.
State Program. The Aged, Blind, and Disabled and/or Covered Families and Children Medicaid Programs. For purposes of this Appendix, State Program may refer to the State agency(ies) responsible for administering the applicable State Program.
State Program. The Commonwealth of Virginia’s Medicaid and CHIP Programs, including the Commonwealth Coordinated Care Plus (CCCP) Program and Medallion 4.0 Medicaid and Family Access to Medical Insurance Security (FAMIS)
State Program. The parties understand and agree that the State Program is separate and apart from the terms and conditions of this Incentive Agreement. For purposes of administration of the State Program, East Fork understands that the Milestone Dates for compliance with reporting requirements with regard to the State Program shall be April 30, 2022 and April 30, 2023. East Fork will cooperate and take all necessary and appropriate actions to assist with County's administration of the State Program. The funds associated with the State program will be disbursed according to the below schedule. Accordingly, East Fork shall indemnify, defend and hold harmless the County and its subsidiaries, divisions, officers, directors and employees from all liability, loss, costs, claims, damages, expenses, attorney fees, judgments and awards arising or claimed to have arisen, from any injury or financial damage caused by, or allegedly caused by, either in whole or in part, any act or omission of the County or any employee, agent or assign of County. East Fork shall indemnify the County in all instances except where the County is primarily negligent through an act or omission or where the County engages in gross negligence or willful misconduct. Nothing herein shall be construed as a waiver on the part of the County to any defense of any claim, including, but not limited to the defense of governmental immunity. And that the County's obligations under this paragraph shall be limited to the extent and manner of recovery pursuant to County's self‐insured claim policies and North Carolina law.
State Program. The QI program offered by the State of Hawaii (“State Program”). For purposes of this Appendix, State Program may refer to the State agency(ies) responsible for administering the State Program.
