Real estate and improvements thereon used exclusively or primarily for the delivery of health or human services, including but not limited to hospitals, clinics, long term care facilities, custodial care facilities (including but not limited to childcare centers), congregate care facilities, assisted living facilities, surgery centers and medical office buildings.
Health Care Facilities. Each of the Shareholders and Health Care Providers maintains in good standing staff memberships or similar affiliations with the health care facilities as set forth on Section 8.27 of the Disclosure Schedule.

Health Care Facilities. The State of Minnesota owns and operates health care facilities located within the borders of the State of Minnesota. The health care facilities are managed by State agencies, the Minnesota Department of Human Services (DHS) and the Minnesota Department of Veterans Affairs (MDVA). Many federal and state agencies provide oversight to the State’s health care facilities, including, but not limited to, the Centers for Medicare and Medicaid Services (CMS), Minnesota Department of Health (MDH), and the federal Department of Veterans Affairs. State Health Care Facilities may be long-term care facilities, hospitals, rehabilitation facilities, or treatment facilities. These State health care facilities serve both adults and juveniles and some of the State buildings are secure and the Contractor may only access through designated entrances and by admittance by a staff member. The Department of Human Services (DHS) Minnesota Sex Offender Program (MOSP) treats 733 individuals between its two sites. The Minnesota Department of Veteran’s Affairs (MDVA) services 800 veterans amongst their homes. As and when appropriate, and as provided hereinafter in this Agreement, to the extent permitted by law, RESIDENTS will have priority access to Rivercrest Wellness and Rehabilitation, a licensed nursing care facility, and The Gardens, a memory support assisted living, managed and operated by and adjacent to NEWBURY COURT.
