Follow-up and Continuity of Care. These services include tracking pending provider and Client results to ensure: pending results are reviewed and documented in Client record; Clients are informed promptly of test results; provision of timely follow-up; compliance with recommended care; return visits; follow-up on missed appointments; and compliance with all applicable state and local laws for disease reporting, includes referral documentation and referral provider.
