Extended Disability Plan. 206 - Eligibility - Regular full-time employees with at least sixty (60) continuous full-time equivalent service months are eligible for extended disability leave. 207 - Usage - In cases which are expected to result in total disability, eligible employees will be granted extended disability leave with full pay and benefits for up to six (6) months from last day worked. This leave will include the aggregate of accumulated sick leave, vacation and personal leave to the extent these benefits are due the employee. This leave will be extended beyond six (6) months until all paid leave accruals have been exhausted.

Extended Disability Plan. 153 Eligibility - Regular full-time employees with at least five (5) years of continuous full-time employment are eligible for extended disability leave. -154 Usage - In cases which are expected to result in total disability (as defined in LTD master contract), eligible employees will be granted extended disability leave with full pay and benefits for up to six months or until all accruals are exhausted, whichever occurs later. This leave will include the aggregate of accumulated sick leave, vacation and personal time to the extent these benefits are due the employee. The total aggregate of all paid leaves, when used for total disability will specifically end in six (6) months or when the total aggregate of all paid leaves is exhausted, whichever occurs later.

  • Extended Disability Leave a. Due to a medical disability, an employee shall be granted an extended leave of absence without pay (subject to Paragraph d hereof) if any one of the following conditions exists:

The employee in receipt of Disability Income benefits under the group plan will have the opportunity to participate during a period not exceeding five (5) years in College benefit plans, as applicable but will be required to pay one hundred percent (100%) of the premium costs thereof. The employee in receipt of Disability Income benefits under the group plan will be entitled to return to the employ of the College during a period not exceeding five (5) years upon submission of a medical certificate indicating fitness for work. Should a single illness exceed ninety (90) calendar days in duration, an eligible employee must apply for disability insurance benefits in accordance with the terms of the plan. Seniority shall continue to accumulate during the period of disability to a maximum of five (5) years.

  • Short Term Disability Plan The administration of the Short Term Disability Plan and the payment of benefits under this Plan shall be handled by the Company.

  • Long Term Disability (LTD 4.7.1 The school board shall cooperate in the administration of the LTD Plan. It is understood that administration means that the school board will co-operate with the enrolment and deduction of premiums and provide available necessary data to the insurer, upon request. The school board will remit premiums collected to the carrier on behalf of the teachers.

  • Short-Term Leave and Disability Plan Top Up (STLDPT) For teacher absences that extend beyond the eleven (11) sick leave days provided above, teachers will have access to a sick leave top up for the purpose of topping up salary to one hundred percent (100%) under the Short-term Leave and Disability Plan. This top up is calculated as follows:

  • Long Term Disability The Employer agrees to provide Long Term Disability benefits for active full-time employees after fifty-two (52) weeks if an Employee is unable to perform any occupation (reasonably suited by means of training, education or experience). The Plan will provide for sixty-six and two thirds percent (66 2/3%) of an Employee's basic monthly earnings to a maximum of $1,500.00. Coverage would cease the date an Employee attains normal retirement age.

  • Long Term Disability Benefit In the event an employee, while covered under this plan, becomes totally disabled as a result of an accident or a sickness, then, after the employee has been totally disabled for seven (7) months, including periods approved in Section 1.3(a) and (c), he/she shall be eligible to receive a monthly benefit as follows:
