Dental Insurance Coverage. Present coverage for preventative care, restorative care, prosthodontics care, and orthodontic care will continue. Coverage will be as follows:
Dental Insurance Coverage. The City shall maintain the current dental coverage for all eligible members, including orthodontia coverage of one thousand eight hundred fifty dollars ($1,850.00). Dental general anesthesia administered by the dentist is a covered service and osseous surgery is eliminated from the dental plan, as this service is payable under the medical plan. A dental PPO shall be available to members which allow voluntary selection of a participating provider which will result in no balance billing over reasonable charges. All existing coinsurance levels and exclusions continue to apply. The following preventative dental services will be paid at 100% of the reasonable charge: • Routine oral examinations – twice in any calendar year, January 1 through December 31. • Routine prophylaxis (cleaning of teeth) – twice in any calendar year, January 1 through December 31. • Topical application of fluoride – twice in any calendar year, January through December 31.
Dental Insurance Coverage. The City shall pay the premium for employee and dependent family dental insurance.
Dental Insurance Coverage a. The board shall provide GESSA employees with the opportunity to purchase either individual or family dental insurance.
Dental Insurance Coverage. The Employer will pay the required dental insurance premiums for each eligible full time employee occupying a job classification covered by this Agreement. Both Class I and Class II Delta Dental benefits will be made available by such insurance, provided, however, such benefits will be on a 50 percent (50%) co-pay basis with a maximum dental benefit of $1,000 in any one (1) calendar year.
Dental Insurance Coverage. The District shall continue to provide the self-insured dental insurance coverage at the same level of benefits and services with a maximum annual benefit of fifteen hundred dollars $1,500 for Premier Dentists and $1,700 for PPO Dentists. This plan runs January 1-December 31.
Dental Insurance Coverage. The Delta Premier Plan option shall be the dental coverage for all eligible employees for individual and dependent coverage.
Dental Insurance Coverage. The City agrees to continue to provide a group dental plan to the employees covered by this M.O.U. The coverage will include the employee and his or her qualified dependents. The carrier of this coverage will not be changed without satisfying the same conditions as provided for a change of the medical insurance carrier.
Dental Insurance Coverage. The Board shall provide GESPA employees with the opportunity to purchase either individual or family dental insurance. The Board shall contribute 90% of the individual dental insurance premiums for each GESPA employee who does not select family medical insurance.
Dental Insurance Coverage. An employee and eligible dependents shall be entitled to dental insurance coverage with monthly premiums paid by METRO. The plan will provide for the following coverages: 100% preventive services, 85% basic services, and 60% major services to a yearly maximum benefit amount of $1,700. For a dependent under the age of 19: 60% orthodontics to a $3,000 lifetime maximum after a $50 deductible.
