Community Centers. Through the procurement conducted in 2017, this provider is eligible to provide services through on behalf of the City to provide nutritious meals to seniors Citywide. City staff have been coordinating the details of the resulting agreements and services since June. However, there have been several changes including the cost of meals, which affect the overall agreement amount. Due to fluctuation in administrative staffing, the changes were not communicated in a timely manner. As a result, the agreement is now in the final stages of department review, signature, and execution. The Department is requesting that the following three agreements be accepted with retroactive provisions. We understand that retroactive agreements are to be avoided. We request an exception be made and the agreement detailed in this memorandum be allowed to proceed with retroactive clauses. Community Centers. The Provider will register with the LEA as a vendor prior to submitting the contract. The Provider will make the initial contact with the parent/ guardian within 10 school days of the on-line enrollment, or contract will be terminated. The Provider shall complete the SLP within 20 school days after receipt of student assessment data, or contract will be terminated. The Provider shall begin tutorial services no more than 10 days after the completion of the SLP or contract will be terminated, and parent/ guardian shall be given the opportunity to choose another Provider if tutoring services do not begin within 10 days of SLP completion. Students’ assessments shall correlate to Sunshine State Standards (SSS). The Provider will not receive compensation for conducting assessments. The School Board will share pertinent assessment information as requested by the Provider pending parental consent to release information. Request shall be made at least one week in advance. The Provider shall adhere to the communication procedures set forth by each school so as to minimize disruption to the regular school day schedule. If the Provider desires to use the District’s facilities must make a separate application for use of facilities through that school’s Principal. The facility usage agreement is a contract within individual schools and the Provider, and is determined by “space available” for The Provider at each school. If space is limited, the Provider who has made application and signed a contract to rent space at that facility first will be considered first. The agreement between the school’s principal and The Provider may be null and void if the availability of space becomes an issue, or if The Provider fails to comply with the facility usage agreement. The Provider shall pay the applicable hourly rate for such use as determined by the District’s current facility use charge requirement. If a Provider is unable to rent space at a school, other facilities outside of the School Board’s facilities that are suitable for delivering instruction to students may be rented by the Provider. The Provider shall provide a written release of liability indicating that the School Board site is not responsible for loss or damage of Provider owned equipment, materials and supplies on school grounds. Community Centers. Community Centers shall include opportunities for office, commercial retail and services and civic uses at a scale that is, for the most, more land intensive than uses in the Neighborhood Center. Residential uses such as multifamily housing, nursing homes and senior housing are also anticipated with conditions to be reviewed under administrative or conditional use permits. Some resource and limited industrial uses shall be allowed with restrictions on size. The Community Centers shall provide services to a customer base from within and outside Tehaleh. A more detailed description of uses allowed in Community Centers is included in Exhibit I.
Community Centers. Developer will deliver one community center within Development Stage One and another within Development Stage Two. Each community center will be approximately 2-4 acres and will offer improvements and programming designed to complement other future community centers planned for areas outside of Development Phase One. Improvements and programming at the community centers are described in the EDC Property Improvements Program (Exhibit H).

  • Community Involvement The Grantee will facilitate and convene a Community Task Force as one means of developing collaboration among the Grantee, affected residents, and the broader community. The Grantee also will provide information to keep the Community Task Force fully apprised of the planning and implementation of revitalization efforts. The Community Task Force shall be comprised of affected public housing residents, local government officials, service providers, community groups, and others. The Community Task Force will provide advice, counsel and recommendations to the Grantee on all aspects of the HOPE VI development process, including shaping the goals and outcome of the Community and Supportive Services Plan. Community Task Force participants also will disseminate information throughout the community about the Grantee's revitalization efforts. The Grantee's responsibilities with regard to the Community Task Force include:
