  • In performing limited verification Entrust (or a subcontractor acting on behalf of Entrust (a "Subcontractor")) may determine whether the organizational identity, address, and domain name provided with Your Certificate Services Application are consistent with information contained in third-party databases (the "Databases"), which may include the Dun & Bradstreet Inc.

  • Your Certificate provides additional information about your Delta Dental plan, including information about plan exclusions and limitations.

  • Upon receipt of Your Certificate Application and payment, Starfield will initiate the necessary authentication procedures to validate Your application.

  • You must, upon such expiration cease all use of Your Certificate Services and remove any Certificates issued under this Agreement it from the devices and/or software in which it has been installed.

  • Starfield will verify information provided as part of Your Certificate request prior to issuing the Certificate.

  • NOTE: Once coverage is lost under Your Certificate, all benefits cease under this Pediatric Vision Care Benefit.

  • Children who are covered under Your Certificate, to age 19, are eligible for coverage under this Pediatric Vision Care Benefit.

Related to Your Certificate

  • Class A-R Certificate The Certificate executed by the Trustee and authenticated by the Trustee or the Authenticating Agent in substantially the form set forth in Exhibit A-R and Exhibit C hereto.
