Transition Plan definition

Transition Plan means a transition plan, acceptable to the LHIN that indicates how the needs of the HSP’s clients will be met following the termination of this Agreement and how the transition of the clients to new service providers will be effected in a timely manner; and
Transition Plan means a written plan for the period of time between an individual's entry into a particular service and when the individual's ISP is developed and approved by the ISP team.   • The Contractor shall submit to CDA a Transition Plan as specified in Exhibit E of this Agreement.
