  • For clarity, it is anticipated that the Transition Coordinators: (i) will meet not more frequently than twice per week during the first two (2) months after the Transfer Date, and (ii) will meet not more frequently than once every two (2) weeks thereafter, and (iii) will not be required to continue in that role beyond the period ending six (6) months after the Transfer Date.

  • The Transition Coordinators shall meet, by telephone or in person, to discuss the status of the transition and to attempt to amicably resolve any disagreements or miscommunications between the Parties that may arise in connection with information to be exchanged, timelines for transfer of documents or information, or the like.

  • In connection with the implementation of each Transition Service, the Transition Coordinators will mutually agree upon what methods are necessary, including, but not limited to, access controls, passwords and other security measures, to effect the purposes of this Section 5.

  • The Transition Coordinators shall meet as expeditiously as possible to resolve any dispute.

Related to Transition Coordinators

  • Services Coordinator means an employee of the community developmental disability program or other agency that contracts with the county or Division, who is selected to plan, procure, coordinate, monitor Individual Support Plan services, and to act as a proponent for individuals with developmental disabilities.
