Testing Request Form definition

Testing Request Form means the request form included as part of the DBS Card contained within the Contract DBS Test Kits and submitted by Physicians directly to Centogene.
Testing Request Form means the request forms included in the Test Kits and submitted by Physicians directly to Centogene.
