Specific Aim. 1: To evaluate the relationship between HIV-positive pregnant women in Zambia and the role and influence that their Christian faith has on HAART adherence. We will hold key informant interviews and surveys with 508 HIV- positive pregnant women as well as 100 identified Christian leaders who actively engage in counseling HIV-positive pregnant women through ministry within Lusaka. These interviews will assess the viewpoints, perceptions and opinions about HAART and how adherence is affected by religion. Specific Aim. 2: Identify overall patient characteristics of CAM users and non- users. There is a paucity of research studies that examine patient perceptions to both CAM and CAM users. It is important to determine who is using CAM before trying to develop theories to explain the growing popularity of CAM use among cancer patients. CAM users and non-users may have significant and important differences that may impact CAM use in the population identified as particularly likely to benefit from CAM. Finding these potential differences will assist in understanding if patient’s perceptions regarding CAM use might limit the uptake of potentially useful CAM therapies. In addition, the research may identify socio-economic, gender, and ethnic groups, also known as “under-represented minorities” (URM) suffering from end-stage cancer that may or may not be using CAM. This information could be valuable in order for the field to be cognizant of who may be using CAM and can provide opportunities for patient education. Specific Aim 3: Assess communication of CAM usage between the patient and the patient’s oncologist, primary care physician or palliative care team. CAM is often used alongside conventional medical care, yet many patients don’t disclose CAM use to their physicians. Serious adverse events are possible due to CAM usage, so it is vital that patient’s teams are aware of exactly what they are using and how often. It is also important to understand the reasons patients do not often disclose CAM usage. The findings of the proposed study will be useful to gain that knowledge, and may assist in creating awareness to these issues. There are future opportunities to incorporate usage of CAM into patient medical questionnaires or other ways of discovering if patients are using any CAM therapies or products. Specific Aim. 1: Design, fabricate, and characterize ultrasound probes for transrectal BH studies. We will simulate nonlinear HIFU fields generated by transrectal probes with different geometries to design a transducer capable of operating in shock-forming conditions relevant to BH. Parameters of FDA-approved HIFU devices (e.g. Ablatherm and Sonacare) will be used as a starting point to meet clinical constraints for prostate treatments and power limitations of the system. Based on design calculations, a transrectal prototype probe integrated with a miniature ultrasound imaging probe will be constructed for ex vivo and preclinical studies in canines. The integrated device will include the imaging system (B&K 3000 commonly used in urology practice), the transrectal HIFU transducer (fabricated by a vendor per our specifications) and the computer-controlled driving electronics (E&I power amplifier and function generator). Acoustic characterization of the HIFU probe will be performed in water, including hydrophone measurements in linear and nonlinear regimes and acoustic holography. The holography measurements will be used to set a realistic boundary condition for rigorous numerical modeling to predict nonlinear acoustic fields and temperature fields in prostate tissue (Xxxxxxx 2015).
