Service Record definition

Service Record means the written or electronic documentation regarding an individual and resulting from entry, assessment, orientation, services and supports planning, services and supports provided, and transfer.
Service Record means an individual, tangible, and legible file that records service-related activities set forth in subrule 152.2(4).
Service Record means the original or a copy of any documentary material, including referral information formal and informal assessments, recommendations, and closing material, for a child receiving service.
