Service Coordinator definition

Service Coordinator has the meaning set forth in Section 2.2.
Service Coordinator means the person with primary responsibility for providing service coordination and care management to STAR+PLUS Members.
Service Coordinator shall have the meaning set forth in Section 2.6.

  • The Service Coordinator will develop and implement the transition plan.

Service Coordinator has the meaning set forth in Section 3.3(a).
