Religious exemption definition

Religious exemption means a vaccination exemption based on an affidavit of exemption on religious grounds attesting that receipt of a vaccine or vaccines is contrary to an individual's religious belief, observance, or practice.

Religious exemption. You declare that immunization for the Select Diseases conflicts with the Student's personally held religious beliefs or practices and request that the Student be enrolled at FSC without immunization.  Medical Exemption: A physician has determined that the Student should not be immunized for the Select Diseases or that the Student has immunity against the Select Diseases. You must attach the physician's signed statement. You will be responsible in those circumstances for any and all costs associated with the Student's exclusion from classes, activities and/or on campus housing, including expenses for relocation, housing, meals and travel. Students excluded from campus or courses due to a vaccine-preventable incident are ordinarily not eligible for a medical withdrawal, course withdrawal or refunds for tuition or other charges at FSC's exclusive discretion. You will also be responsible in the event another Student or an employee or agent of FSC contracts the Select Disease(s) and for the expenses that any such person or third party incurs or will incur as a result of Your decision to exempt the Student from vaccination.
