Examples of Refund Form in a sentence

  • If you terminate the Agreement at any time after the Occupancy Date, we will pay the 90% Refund Amount within sixty (60) days of the date that you become eligible for refund from the Refund Account per section 7.6. We will pay the 90% Refund Amount to the duly designated beneficiaries named in your Refund Form or, if there is no Refund Form, then to you as the Resident.

  • Policy form means the form on which the policy is delivered or issued for delivery by the issuer.
