Proposal Form definition

Proposal Form means that certain Part C of the RFP, completed and executed by the Proponent, to which this Appendix 1 is appended.
Proposal Form means the form contained in Part C of the RFP.

Proposal Form means the Policyholder’s statements in the proposal for this Policy submitted by or on behalf of the Policyholder along with any other information or documentation provided to the Company prior to inception.
